The Framework I Actually Use In The Clinic
The Simple Model Behind Every “Complex” Case I Take
In the last post I talked about people who are stuck at 50–70% better.
They’ve done a lot of the “right” things, but they don’t have a clear way to:
Prioritize their efforts
Know what matters most right now
Decide what not to do
If you’ve ever felt like that, this is the model I wish someone had given you 10 years ago.
Repair Deficit: The Real Problem
Underneath almost every chronic and autoimmune‑type case I see is the same process:
Repair deficit – damage is accumulating faster than your body can repair it.
It shows up as:
Chronic fatigue
Brain fog
Pain and inflammation
Gut issues
Strange, shifting symptoms that never quite resolve
You can call the end result whatever you want. It turns into many “labels” - Hashimoto’s, IBS, Chronic Fatigue, “mystery illness” - but the process is the same.
In our practice, I walk patients through four elements of our framework:
Principles - the attitude, mindset, and outlook that leads to wellness
Essentials – breathing, hydration, light, food, movement
Hazards – things that destroy your body (toxic metals, artificial light at night, processed food, electrosmog, etc.)
Diagnostics – how we measure repair deficit and guide decisions
Therapeutics – tools we use to accelerate repair (medications, supplements, light therapy, etc.)
Most people are drowning in Therapeutics.
They’re arguing about the 17th supplement while ignoring the fact that with a few tweaks to their diet, they could get as much or more nutrition from their daily food intake.
We see this all the time, even in our most well-read, well-researched, motivated, and intelligent patients. It’s not due to a lack of effort, diligence, or motivation, it’s due to a lack of a structured, systematic approach to wellness.
The Order Matters
This is important:
You don’t start with fancy lab panels.
You don’t start with high‑dose protocols.
You start with Essentials, clean up obvious Hazards, then use Diagnostics to decide which Therapeutics are worth it.
In our practice, we run labs immediately while we work on implementing key habits and eliminating hazards.
The thing I want everyone to know, whether they ever work with us or not, is that simple daily habits and choices are often what make or break success.
Not fancy lab panels.
Not high-dose protocols.
Not crazy biohacks.
Simple, ordinary habits and choices.
That’s exactly how I approach complex real‑world cases:
Which Essentials are missing?
Which Hazards are obvious?
What Diagnostics do we need to stop guessing and get clarity on what to do next?
What Therapeutics should we add right now?
In our practice, we’ve built many protocols and tools to help our patients succeed.
But the most important thing is that the framework is simple enough to remember and powerful enough to work in different diseases.
The Immune Wellness Program soon… we’re putting the final finishing touches on it.
I hope to see you on a live call soon.
Until next time, be well,
Dr. Stillman
